Living with a long-term condition can sometimes make it difficult to manage day-to-day life. So it's important that you get the right information and the right help at the right time. This online information produced by the NHS Choices team will guide you through the choices available for self care (or self management). As a minimum, the support to help you manage your own care should include choices in the following areas.
