Integrated care pilots

The 16 pilot sites that have been selected to be part of the programme of Integrated Care Pilots were announced on 1 April 2009. Download details of selected sites for Integrated Care Pilots (PDF, 80K).

The pilot sites at Bournemouth and Poole Teaching PCT and Cornwall and Isles of Scilly PCT will both focus on dementia.

The pilots which will run for two years and will be evaluated over three years against a set of national and local measures, will test a number of diverse models, focussing on innovation and delivering an improvement in outcomes, quality, and patient satisfaction.

Ultimately, every locality should seek to have a single community-based support system focussed on the health and well-being of the local population. Binding together local government, primary care, community-based health provision, public health, social care and the wider issues of housing, employment, benefits advice and education/training.

  • effective processes for assessment and decision-making
  • fast delivery of whatever help is needed
  • reduced risk of errors and unnecessary stages in the process
  • service users in control of the care they receive
