The Department of Health and the South West Development Centre jointly commissioned a project to look at the transition issues for young people as they move into and out of the secure estate. The Project focused on those young people with a Learning Disability, with Mental Health issues and those needing support with Substance Misuse issues.
The Project is complete and we would now like to invite you to the launch of the Transition Care Pathway, where we also would like to consult on the implementation of the commissioning recommendations in the Project Report.
The aims of the day are to:
- Be refreshed on the policy and guidance related to these young people
- Hear the Project Team present their findings
- Have the opportunity to discuss the stages of the Care Pathway
- Hear the recommendations for service commissioning
- Discuss the current practice gaps and ways to overcome them
- Develop an implementation framework for the south west
The event is intended for a very wide range of colleagues:
- Directors of Children’s Services Local Authority Heads of Social Care
- Directors of Public Health Local Authority Commissioners
- PCT Directors of Commissioning YOT Teams
- PCT commissioners for Children’s Services, Substance Misuse, Mental Health and Learning Disability
