Commissioning personalised care planning for people with long term conditions
This resource pack has been put together as a direct result of the NHS South West Red / Amber / Green assessment carried out across the region in July 2009. It brings together information about personalised care planning, relevant documents and information from the Department of Health and elsewhere to give you all you need to develop personalised care planning in your locality.
For each section there are key actions to take as part of the process of developing and implementing personalised care planning and monitoring and evaluating the outcomes.
